Check if your health practitioner is qualified, registered and their current registration status

Supporting Documentation for Graduate Applications

The online application form will provide you with a list of documents that you will need to scan then upload before you can submit your application.

All uploaded documents must:

  • be in colour, clear and legible 
  • be clearly titled in English (no special characters) 
  • be in one of the following formats: JPEG, BMP, GIF, PNG, PDF, WORD, or TIFF 
  • be unencrypted (e.g. an unencrypted PDF) 
  • be an uncompressed file 
  • have a minimum resolution of 300 dpi, and 
  • be smaller than 10MB in file size.

Any documents containing a photograph (e.g. a passport, driver’s licence, etc.) must be certified by an authorised officer with the statement ‘I certify that this is a true copy of the original and the photograph is a true likeness of the person presenting the document as sighted by me.’

We may, at any time, request for the documents you upload with your online application to be provided in certified hardcopy via post.

Summary of online requirements

1. Create your Ahpra account

You will be prompted to create an Ahpra account so you can save and resume your application at any time. You must ensure that the email address you use is only accessible by you and must not be shared between another applicant or currently registered practitioner. We will also use this email address to contact you if we require further information to support your application.

2. Proof of identity

You must provide sufficient evidence of your identity.

3. Proof of change of name

If you provided one or more previous names in your online application, you must upload copies of your name change documentation. The name on your application for registration must match the student name provided by your education provider. If your name has changed after enrolling in an approved program of study, you must contact the education provider to update your details before you are due to finish your program of study.

4. Your graduate results

Your education provider will provide your graduate results directly to us. You are not required to upload your academic qualification, however, if you have received your certificate please upload a copy with your application for us to retain on your record.

5. English language skills

To be eligible for registration you must provide evidence of English language skills that meet your profession’s English Language Skills Registration Standard.

Please note, if you meet the requirements of the Primary language pathway or Combined secondary and tertiary education pathway and have completed an approved program of study, you are not required to provide supporting evidence of English language skills. You will be required to complete the English language skills requirements table in the online application form.

6. Criminal history

If you disclosed a criminal history in Australia and/or overseas in your online application, you must provide details of the circumstances in a written statement with your application. If you have any documents available to support your statements, you will be prompted to upload these with your application. Your profession’s National Board will decide whether your criminal history is relevant to your ability to practise your profession.

If you disclosed a criminal history outside of Australia and/or you have lived overseas for six consecutive months or longer from the age of 18, you will need to arrange for an international criminal history check (ICHC) from an Ahpra-approved supplier and provide us with the reference details. The supplier will provide the results of the check directly to us.

7. Impairments

If you disclosed an impairment in your online application, you must provide a written statement with details of that impairment and how it is managed with your application. If you have any documents available to support your statements, you will be prompted to upload these with your application. Your profession’s National Board will decide whether your impairment is relevant to your ability to practise your profession.
